Your form submission is stored in our practice management platform, which is a service provider contractually bound to protect your information. Information may also be shared with our electronic health record and scheduling systems once you become a patient. We collect consumer health data only when you provide it through the form, and only to respond to your request for care. We do not sell consumer health data. We do not use geofencing around health facilities.

Email, text messages, and website forms are convenient but not fully secure. Please do not use this website to send detailed clinical information, medication questions, or anything time-sensitive. Once you are a patient, use the secure patient messaging platform we provide.
This site is intended for adults and for parents or guardians requesting care for a teen. If you are under 18 and want to request care, please have a parent or guardian submit the form, or contact us directly by phone.
Form submissions from people who do not become patients are kept for 12 months and then deleted. Information about patients is retained as part of the medical record according to Washington law and our Notice of Privacy Practices.
