Information you give us. When you submit the appointment request form or contact us, we receive what you enter: typically your name, email address, phone number, whether you are located in Washington, your insurance plan, and a brief description of what you're looking for.

Communication

Email, text messages, and website forms are convenient but not fully secure. Please do not use this website to send detailed clinical information, medication questions, or anything time-sensitive. Once you are a patient, use the secure patient messaging platform we provide.

Children

This site is intended for adults and for parents or guardians requesting care for a teen. If you are under 18 and want to request care, please have a parent or guardian submit the form, or contact us directly by phone.

Data retention

Form submissions from people who do not become patients are kept for 12 months and then deleted. Information about patients is retained as part of the medical record according to Washington law and our Notice of Privacy Practices.

Changes

We may update this policy. The effective date at the top will change when we do, and the current version will always be posted here.
